The BBC was found to have committed a "significant breach" of media regulations by failing to disclose that the presenter of a film about Gaza was the offspring of a Hamas administrator, according to UK media regulator Ofcom.
An inquiry by Ofcom into Gaza: How to Survive a Warzone has found that the programme was "substantially inaccurate".
The chief executive of the broadcaster had already issued an apology, stating there had been "a serious error in relation to accuracy".
Ofcom has ordered the broadcaster to broadcast a evening statement about its conclusions.
"Since this amounts to a major infringement of our rules, we are directing the BBC to transmit a announcement of our determinations against it on BBC2 at 21:00, with a schedule to be finalised," it said.
'Potential to Erode Trust'
The regulator's statement declared: "The inquiry we conducted determined that the documentary's failure to disclose that the voice's parent served in a post in the Hamas-controlled government was materially misleading.
"This implied that the public did not have vital facts which may have been very significant to their judgment of the narrator and the details he presented.
"Credibility is at the heart of the bond between a television network and its audience, particularly for a state-funded media outlet such as the BBC.
"This error had the potential to erode the significantly high levels of trust that the public would have invested in a factual film from the broadcaster about the Gaza conflict."
The documentary was removed from iPlayer in February after the boy's familial connections were revealed, and in the month of July an internal BBC review found it violated the organization's editorial guidelines on truthfulness.
A BBC spokesperson said: "The regulator's decision is in accordance with the findings of the internal assessment, that there was a major oversight in the film in relation to the BBC's Editorial Guidelines on factual correctness, which aligns with regulation 2.2 of Ofcom's Broadcasting Code.
"We expressed regret for this and we agree with the authority's determination in complete. We will adhere to the penalty as soon as the date and wording are confirmed."
